ABOUT ALTITUDE CARES

Altitude Cares, a CMS Access Program Participant, is an AI-powered medical group that provides post-discharge and between-visit chronic care management. It proactively follows patients from the point of referral through ongoing care management. Clinician teams, supported by Altitude's platform, take continuous ownership of medication reconciliation, lab follow-through, titration, and care coordination from the moment a patient leaves the hospital or is flagged by their outpatient provider.

Referrals come from two channels: at discharge from hospital, ED, or SNF; and from outpatient specialists and PCPs managing patients with poorly controlled chronic conditions. Once referred, Altitude reaches the patient within 24 hours, completes medication reconciliation by Day 2, conducts a telehealth visit by Day 7–14, and maintains continuous management for as long as clinically needed. Referring providers receive structured updates when — and only when — something changes in management.

Altitude operates care management programs across Rhode Island, Virginia, and other markets. Indiana is the next expansion — bringing direct clinical capacity to busy practices whose patients fall through the gap between visits.

ROLE OVERVIEW

The advisor’s job is to develop Altitude Cares’ Indiana referral network by introducing the services and benefits to discharge teams, specialists, and medical groups serving patients with chronic conditions.
